I acknowledge that under Arizona law, I am required to remain physically present at Broken Clover Tattoo for the entire duration of the procedure involving the minor. I further acknowledge that any change to placement, design, or procedure after this consent is submitted requires a new consent form to be completed and signed before work may continue. 

I acknowledge that if I leave the premises or fail to remain physically present for the duration of the procedure, Broken Clover Tattoo reserves the right to immediately stop all work on the minor without refund. I further acknowledge that any injury, complication, or dispute arising from my absence or failure to comply with Arizona law will be my sole responsibility, and I agree to indemnify and hold harmless Broken Clover Tattoo, its artists, owners, and employees from any resulting claims, damages, or legal action.

I understand this service is a free tattoo touch-up only and does not include additional design changes or new tattoo work.

• I acknowledge that the price listed on this form is the total amount I am obligated to pay for this service. Broken Clover Tattoo will not demand payment beyond the amount listed here. I understand that any deposit paid is held by the individual artist performing the service. If the artist fails to deliver the agreed service, the artist is responsible for making the customer whole. If the artist is no longer working at Broken Clover Tattoo at the time of dispute, I understand the shop may provide documentation or testimony in small claims court to help establish the facts of the transaction, but is not financially responsible for the artist’s independent obligations.

I confirm that all information provided in this form is true and complete to the best of my knowledge. I acknowledge that I have read and agreed to the procedure details, pricing terms, responsibility statements, and risk acknowledgments in this consent form. I understand that tattooing and piercing involve inherent risks, that results vary by individual, and that proper aftercare is my responsibility. If signing as a parent or legal guardian for a minor, I certify that I have the legal authority to consent and agree to remain physically present for the duration of the procedure as required by Arizona law. I understand that this submission constitutes my legally binding electronic agreement. I agree that any dispute arising from this procedure or agreement will be governed by the laws of the State of Arizona, with jurisdiction and venue in Arizona courts.
